This site provides a dedicated platform for Designated Safeguarding Leads to register as new Key Adults and Headteachers to complete their Information Sharing Agreement. Once you have completed the online form, you can download a PDF of your Key Adult Agreement form or Information Sharing Agreement.

If you wish to update your existing Key Adult Agreement information such as new surname, email change please again complete the key adult  form electronically.
